15.1 Procedure for Placement
The Children’s Service Worker will complete the CS-10 according to directions. The CS-10 is the tool used by the worker to assess the need of a foster child for medical foster care (MFC), and to refer that child, through supervisory channels, for approval. This form should be used, in conjunction with other appropriate information, to document the child’s eligibility for MFC. The worker will use their knowledge of the child, information supplied by the birth family, the resource family, other professionals, and/or the child to realistically assess the needs of the child. Care should be taken to make the information on the CS-10 understandable to the staff person reviewing it. The more comprehensive the documentation, the more likely a decision can be made as to the eligibility of the child.
NOTE: The form shall be completed based upon the child’s disability and not his/her age.
15.2 Qualifications of Medical Foster Home
The Children’s Service Worker will refer to the contract section of the Forms Manual, CM-3 MDFC for instructions to amend the cooperative agreement of a specific kinship home, licensed resource family home, or resource family group home to provide for medical foster care payments.
Within the contract amendment to provide medical foster care, the foster parent(s) agrees:
- To provide care to children with acute medical problems or severe physical/mental disabilities who have been determined eligible for medical foster care by the Division;
- To obtain and maintain, during the placement of such children described above, foster parent liability insurance;
- To work closely with, and accept special training from, the health care professionals attending the child in placement; and
- To provide transportation for children in medical foster care to and from treatment facilities.
The Division agrees to reimburse the foster parent(s) for maintenance for children approved for medical foster care at the rate of six hundred fifty-seven ($657.00) dollars per month per child. The amount of maintenance reimbursement shall be pro-rated if a child is in care for less than a calendar month. Special expenses, if any, shall be paid in accordance with Division policy.
This reimbursement for children in medical foster care shall be in lieu of the reimbursement provisions in paragraph #17 of the cooperative agreement.
