The purpose of this memorandum is to inform staff of the revisions to the Referral/Information for Child Support Services (CSE-201) form, CSE-201 instructions, and a policy change when Child Support (CS) refers a Temporary Assistance (TA) case for sanction. The most recent revision date of the form is August 2012 (8-12). Revisions include a name change of the form from Child Support Enforcement (CSE-201) to Child Support (CS-201) and the addition of the FSD Intake Center's mailing address at the bottom of the last page of the form.
This memorandum contains information on the following:
- PURPOSE OF THE CS-201 FORM;
- MAILING ADDRESS;
- ORDERING INFORMATION;
- TA SANCTION POLICY CHANGE; AND
- MANUAL UPDATES.
PURPOSE OF THE CS-201 FORM
The CS-201 form provides information on the non-custodial parent. The CS-201 is completed and forwarded to CS for TA and MO HealthNet (MHN) cases when caretakers are required to cooperate with CS in the pursuit of child and medical support.
Effective immediately, direct all mailings of the CS-201 (or CSE-201) to the FSD Intake Center's mailing address:

At the discretion of the Income Maintenance (IM) Family Support Division (FSD) office, staff may mail the completed CS-201 form to the above address or scan and e-mail the completed CS-201 form to [email protected].
Do not destroy/recycle old versions of the CSE-201 form. Staff are instructed to continue to use the CSE-201 forms currently available in the FSD offices and order from the warehouse as needed.
There are 22,000 copies of the August 2006 (8-06) CSE-201 version located in the warehouse. Once this supply is depleted, the new CS-201 (8-12) version will be available.
The revised CS-201 is uploaded and available in the FS Forms file located on the computer desktop.

TA SANCTION POLICY CHANGE
Effective immediately, sanction requests from CS will contain the word "Sanction" in the subject line as opposed to previous TA policy introduced in Income Maintenance memorandum IM-#55 Tracking Temporary Assistance Sanction Requests from Child Support, dated July 3, 2012 that stated the wording "TA Sanction" would appear in the e-mail subject line.
Several sections of the Income Maintenance Manual have been revised. Sections 0205.060.05; 0205.060.10; 0205.060.15; 0205.060.25; 0905.010.40; 0905.010.40.05; 0905.010.40.10; 0920.010.25; 0920.10.30; 0920.010.30.05; 0920.010.30.10 are updated to include the name change of the CS-201, the new CS centralized mailing address for the CS-201, the option of scanning and emailing the CS-201 (CSE-201), and removal of the option to "hand-deliver" CSE-201s. Faxing continues to remain an option for forwarding the CS-201 (CSE-201) forms. When faxing completed CS-201s (CSE-201), fax to 573-635-7545.
Section 0205.060.35 is revised to include the TA policy change for sanction requests.
- Review this memorandum with appropriate staff.
- Order the CS-201 as needed from the warehouse using normal ordering procedures.
- Direct all mailings of the CS-201 to the new centralized mailing address.
