Remittance Advice Remark Codes and Claim Adjustment Reason Codes

With the implementation of HIPAA national standards, previously used MO HealthNet edits and EOBs will no longer appear on Remittance Advices.

Instead, HIPAA compliant Remittance Advice Remark and Claim Adjustment Reason Codes are used.
